I believe that Ben fell out of favor with Jacob and the island; that is the reason why he got the cancer on his spine. Well, that is what Ben said to Locke when they found Jacob's cabin again. "John, the island wanted you to get well, and me to get sick" is what he told John when he refused to go inside the cabin. Ben also knows that Locke can hear Jacob. Thats why he shot John at the end of season 3.
